All individuals who complete any of the   application forms or feedback forms on the   avph. by, confirm that they have fully read and agree with the terms of this Consent and Privacy Policy. You as a potential client of Arvitfarm LLC (hereinafter —Client), agree to:
Consent to processing of personal data
1. Hereby, the Personal Data Subject, in accordance with the requirements of Article 5 of the Law of the Republic of Belarus dated  7 May 2021  No. 99-Z “On the protection of personal data”, confirms its unequivocal, informed and conscious consent to the processing of personal personal data by Arvitfarm LLC, located at   220 037,   Minsk, per.   Kozlova 7d , office 17 with use of automation tools and (or) without the use of such tools in according to the terms of this agreement.
2. Consent is given to the processing of the following personal data:
2.1. surname, name, patronymic (if any);
2.2. E-mail address;
2.3. contact phone number and information about activity on the site, i.e. e-mail address, phone number (mobile), to which the Personal Data Subject has provided access, as well as information that the Personal Data Subject has published by making publicly available.
3. The processing of personal data means any action or set of actions performed with personal data, including collection, systematization, storage, modification, use, depersonalization, blocking, distribution, provision, deletion.
4. Purpose of personal data processing:
4.1. ensuring communication between the Personal Data Subject and  LLC Arvitfarm, necessary for:
— fulfillment by Arvitfarm LLC of its obligations to the Personal Data Subject;
— feedback forms on the website of OOO Arvitfarm;
—  making telephone calls to the Personal Data Subject in the framework of the fulfillment of Arvitfarm LLC’s obligations to the Personal Data Subject;
4.2. processing by  Arvitfarm LLC of requests and requests received from the subject of personal data;
5. Users (persons authorized to  process) the personal data of the Personal Data Subject are employees of Arvitfarm LLC.
For the purposes of processing personal data set forth in clause 4 of this consent, LLC Arvitfarm has the right to engage third parties on conditions that these persons comply with the requirements of the legislation of the Republic of Belarus on personal data. At the same time, Arvitfarm LLC guarantees the Personal Data Subject to ensure the confidentiality and security of his personal data.
6. The subject of personal data is aware of:
6.1. the rights of the Personal Data Subject related to the processing of personal data, and the mechanism for exercising such rights, in particular:
— the right to withdraw consent to the processing of personal data (at at any time without giving reasons);
— the right to to receive information regarding the processing of their personal data;
— the right to change your personal data;
— the right to receive information about providing your personal data to third parties;
—  the right to demand termination of the processing of personal data;
— the right to demand the deletion of their personal data;
— the right to appeal against actions (inaction) and decisions of Arvitfarm LLC related to the processing of personal data;
These rights are exercised by the Personal Data Subject by submitting an application to Arvitfarm LLC in written form or in the form of an electronic document.
6.2. the consequences of giving consent by the Subject of personal data or refusal to give such consent.
7. The subject of personal data has the right to appeal against the actions (inaction) and decisions of Arvitfarm LLC that violate his rights in the processing of personal data to the authorized body for the protection of the rights of personal data subjects.
8. The subject of personal data is familiar with:
— with Policy regarding the processing of personal data of Arvitfarm LLC:
— consent to processing of personal data is valid from the date of its expression and until achievement of the purposes of processing personal data or until its withdrawal.
— consent to processing of personal data can be withdrawn by submitting a written application to LLC Arvitfarm sent to the following postal address: 220 037, Minsk, lane  Kozlova 7d, office 17 or   as an electronic document to   e-mail address: info@avph.by;

The Personal Data Subject hereby confirms his consent and permission to processing his personal data on the above conditions, and also confirms that before providing this consent he is simply and clearly explained his rights related to processing of his personal data, mechanisms their implementation, and the consequences of giving or refusing to give such consent.
